Financial and CANcellation Policy

  • We accept Visa, Mastercard, Discover, cash or check. We offer third party financing for qualifying patients through Care Credit.  Depending on your credit score, different options are available to you with varying interest rates. 
  • We accept ALL insurance policies that have both in and out of network coverage. It is the patient’s responsibility to research and be familiar with their policy and benefits.  Dental insurance is a contract between you and your insurance company. 
  • We will file all insurance claims as a courtesy so it is important to provide accurate and complete information. While we will submit claims to your insurance company on your behalf, we are not privy to their decision making and we will direct you to contact your insurance company if you have any questions or concerns about insurance coverage
  • We require a 50% deposit to schedule a surgical procedure.  
  • We require a 50% deposit to schedule a scaling and root planning (deep cleaning) procedure.
  • Payment in full/co-payments/deductible(s) are due at the time of service.
  • Refunds/overpayments are mailed by check to your address on record within 30 days of a credit being generated on your account.
  • Any returned refund checks and/or bounced checks are subject to a $5 reprocessing fee.
  • Failure to pay your account balance in full will result in turning your balance over to an outside collection agency.

No Show/Cancellation Policy

  • I understand I have a minimum of 48 hours prior to my appointment to notify of changes or cancellations. If I fail to do so, I am subject to a “No Show/Late Cancellation” fee of $125.00.
  • Appointments scheduled on Monday will need to be cancelled/rescheduled by 5:00 pm the Wednesday prior.
  • For cancelled surgery appointments with less than a 72-hour (3 business days) notice, I will forfeit the 50% deposit that was paid to reserve my appointment.
  • As part of our normal patient confirmation process, patients will receive several text messages, emails and or phone calls to confirm their appointment. I am required to respond to appointment confirmation attempts via e-mail, text messages and phone calls. 
  • If I do not respond to confirmation attempts, we are under the assumption that you will not be attending your appointment and your appointment may be cancelled without notice.
